Defination:

Gonorrhoea is an infection of the mucus membrane of genitourinary tract caused by Neisseria Gonorrhoeae

Incubation Period:(The interval between exposure to infection and appearence of the first symptom)

2 to 10 days

Causes:

Gonorrhoeae is acquired during sexual act but sometimes it may occur from contact with infected bed covers,towels,handkerchiefs etc.

Symptoms:

SYMPTOMS IN MALES:

In males infection first starts from the anterior urethra and gradually spreads to prostate,urinary bladder and epididymis.Due to involvement of these organs different features will develop 2 to 5 days after sexual contact.

Symptoms Related To Urethra (Canal for passage of urine):

  • Burning sensation during micturition (while passing urine)
  • Dysuria (painfull passage of urine)
  • White or greenish yellow or yellow discharge from urethra which will be less and less gradually and will be observed only on waking up from sleep which is called gleet.
Symptoms Related Prostate (A gland that surrounds neck of the bladder and the urethra in males):

  • Retention of urine.
Symptoms Related To Seminal Vesical (Sac like structures producing thick viscous fluid that forms a part of semen):

  • Pain
  • Local Tenderness
  • Fever with chill
Symptoms Related To Urinary Bladder:

  • Frenquency of micturition (passes urine again and again)
  • Strangury (Painfull and interrupted urination in drops)

Symptoms Related To Epididymis:

  • Severe pain
  • Swelling
  • Tenderness
Symptoms Related To Testicles:

  • Pain
Symptoms Related To Eyes:

  • Conjuctivitis
  • Panophthalmitis
SYMPTOMS IN FEMALES:

In females the infection is limited to urethra,cervix,fallopian tubes,bartholins glands.

In many cases the disease runs an asymptomatic course.Hence the female acts as carrier.


Symptoms Related To Urethra (Canal for passage of urine) In Females:

  • Frequency Of Micturition (passes urine again and again)
  • Pain 
Symptoms Related To Vagina:

  • Vaginismus (Painful spasm of vagina from contraction of the muscles surrounding the vagina)
  • Painful coitus (Painful sexual intercourse)
  • Vaginal Discharge
Symptoms Related To Cervix:

  • Cervicitis (Inflamation of the cervix)
Symptoms Related To Bartholin's Gland:

  • Bartholinitis
  • Painful swollen glands on two sides of labiae
  • Ultimately abscess may develop
Symptoms Related To Fallopian Tubes:

  • Lower abdominal pain
  • Fever
  • Tender mass seen during pelvic examination
  • Backache
Symptoms Related To Eyes:

  • Conjuctivitis
  • Panophthalmitis
FINDINGS ON EXAMINATION:

IN MALES:

  • Local lymph nodes in the groin are enlarged and tender
  • The margin of external urethral meatus is red and oedematous
  • The urine is hazy and in two glass test second sample is clearer than the first one
  • Due to toxic effect there may be rise in temperature,tachycardia (heart rate over 100 beats per minute in adults),headache and malaise.
Investigations:


  • Urethral discharge on examination will show gonococci which are gram negative and intracellular.
  • Culture test from urethral discharge or urine culture can be performed
  • Complement fixation test for gonorrhoea may be positive

ALLOPATHIC TREATMENT:

Following antibiotics are usefull in treatment of gonorrhoea they can be used under supervision of registered medical practitioner MBBS,MD ETC.

  • Penicillin
  • Tetracycline
  • Ciprofloxacin
  • Ofloxacin
  • Cefixime
  • Erythromycin
  • Azithromycin
  • Doxycycline

Vati:

  • Marma gulika
  • Shilajitadi vati
  • Chandanadi vati(used in chronic condition)
  • Shatpatrayadi vati
  • Chandarprabha vati (used in chronic condition)
Choorna:

  • Chopachini choorna (used in chronic condition)
  • Chandanadi choorna
Ghrita:

  • Shata douta ghrita
  • Vasthya mayantaka ghrita (used in chronic condition)
  • Salmali ghrita (used in chronic condition)
  • Varahayadi ghrita (used in chronic condition)
Lehya:

  • Shatavari lehya (it is to be taken after meals before bed time) (Also used in chronic condition)
  • Amrasa rasayan (used in chronic condition)
Asav/Arishta:

  • Chandanasav (used in both acute and chronic conditions)
  • Sarivadyarishat (used in both acute and chronic conditions)
Bhasma/Kshaya:

  • Shring Bhasma (used in chronic condition)
  • Vanga bhasma (used in chronic condition)
  • Naga bhasma (used in chronic condition)
  • Yasada bhasma (used in chronic condition)
  • Trivanga bhasma
Rasa kalpana:

  • Piyumehantaka Rasa
  • Bhritvangeshvara Rasa (used in chronic condition)
